Westbrom v Chelsea: The Blues on the verge of history

Chelsea will go to Hawthorns this evening hoping to seal the 2016-17 Premier League title by beating Westbrom in front of their fans.

The London club have won just one of their last five fixtures against Westbrom at Hawthorns but this evening Conte and his men will be doing everything to finish on high.

• A Chelsea win would secure their fifth Premier League title in the last 13 seasons, a record only matched by Manchester United.
• Victory would also give them 42 points away from home this season, which would be the most by any side since Manchester United recorded the same total in 2012.
